
The team at Pasadena Medical has been recognised with a Health Equity Award for their work improving chronic condition management and health equity through community engagement.
Supported by Adelaide PHN’s Flourish Primary Care Innovation Grant, the project was led by Practice Manager Rachel Morgante and Nurse Manager Meg Hall.
The initiative focused on improving access to chronic disease management for culturally diverse communities, Aboriginal and Torres Strait Islander peoples, and people experiencing socioeconomic disadvantage.
Through tailored care planning and community engagement, the team is helping patients better understand and manage their health, while increasing engagement with essential healthcare services.
